Who operates this site

Surge Basketball is operated by Jake Tillery in Northeast Arkansas. Privacy questions and parent requests may be sent to neasurge@gmail.com or made by calling 870-450-8724.

Information we collect

Our AAU and Training forms may collect a parent or guardian’s name, email address and phone number, plus an athlete’s name, age or grade, school, program interest, availability, goals and other information the parent chooses to provide.

How we use it

We use submitted information to review registrations and training requests, place athletes appropriately, communicate schedules and program information, manage attendance and follow-up, and send approved payment instructions. We do not sell personal information.

Children’s information

These forms are intended to be completed by a parent or legal guardian, not by a child. A parent or guardian must approve the submission of an athlete’s information. Please do not submit medical, financial or other sensitive information through these forms.

Where information is stored

Form responses are stored in private Google Sheets accessible to authorized Surge Basketball personnel. Payment information will be processed by Stripe after payment service setup; Surge Basketball’s website forms do not collect full card numbers.

Sharing

Information may be shared only with service providers used to operate registrations, communications and payments, or when required by law. Those services have their own privacy and security practices.

Retention and parent choices

We keep information only as long as reasonably needed to operate programs, maintain appropriate records and meet legal obligations. A parent or guardian may ask to review, correct or delete their family’s submitted information by contacting us. Some information may be retained when required for legitimate business or legal purposes.

Security and updates

We use reasonable administrative and technical safeguards, but no online system is completely secure. We may update this notice as our forms, payment process or legal obligations change. The effective date above will be updated when material changes are made.
